Forms and Templates for External Practitioners

There is a requirement for all practitioners employed by Cafcass to use the forms and templates provided. To access them, please click on the links below.

If you wish to view templates with embedded guidance on how best to use them or you are unsure of what template to use in a case then please discuss with your Cafcass manager.
